Welcome to the CIDA Online Faculty Center. Please read all of the information below before logging in at the bottom of the screen. For your convenience, CIDA has implemented an Online Faculty Center, which will allow you to: update your contact/professional profile; complete required CME forms; upload abstracts; request hotel accommodations (if eligible) and view your speaking responsibilities. CONFIRMATON OF PARTICIPATION (DUE UPON LOGIN)

Upon login to the CIDA Online Faculty Center, please be sure to indicate whether your able to participate as a faculty member. To facillitate communication and ensure accurate course materials please update your professional and contact information as well.

FACULTY DISCLOSURE FORM (DUE UPON LOGIN)

In accordance with ACCME standards, all invited faculty must complete, as soon as is possible, the disclosure form found in the online faculty center. Faculty participation cannot be confirmed until the requisite CME forms have been completed.

FACULTY W-9 FORM (DUE UPON LOGIN)

Course faculty are required to print, complete and return the W-9 Form provided via the Online Faculty Center. International faculty are not required to submit a W-9.

FACULTY ABSTRACT SUBMISSIONS (DUE NOT LATER THAN JULY 15)

Course faculty are asked to submit an abstract/short paper for each of their assigned presentation(s) by July 1, 2010 so that quality course materials can be produced for meeting attendees. Abstracts should be uploaded through the Online Faculty Center and will be published in the Journal of Vascular Access (JVA). Parameters have been inserted below.

• Text of Abstract (500 Words, in 3-7 Paragraphs)
• Optional Table or Figure
• 3-7 Properly Formatted References ( See JVA Instructions to Authors for Reference Format)
